  • SCOPE OF SERVICES: Durban Dental Care agrees to provide dental services to the patient as agreed upon and necessary for the patient’s oral health.

  • PATIENT RESPONSIBILITIES: Patients agree to provide accurate medical history information, disclose any pre-existing conditions, medications or allergies that may affect dental treatment, and follow the practitioner’s instructions for pre and post treatment care. Should you fail to disclose this information, the practitioner cannot be held responsible for consequences that may follow in relation to the above mentioned.

  • APPOINTMENT SCHEDULING AND CANCELLATIONS: Patients are responsible for scheduling appointments in advance. Cancellations or rescheduling must be made at least 24 hours in advance, except in cases of emergencies. Failure to provide adequate notice for cancellations may result in a cancellation fee.

  • FEES & PAYMENTS: The patient agrees to pay for services rendered at the time of treatment unless other arrangements have been made in advance. Fees for services are determined by Durban Dental Care, and will be communicated to the patient prior to treatment. Payment methods accepted include cash, credit/debit cards, medical aid and medifinance (Medifin). Any outstanding balances not covered by medical aid are the responsibility of the patient and must be settled promptly.

  • TREATMENT PLANS: Durban Dental Care will create a treatment plan based on the patient’s oral health needs and discuss it with the patient before initiating treatment. Any changes or modifications to the treatment plan will be communicated, including impacts on cost and outcomes.

  • EMERGENCY CARE: Durban Dental Care will make reasonable efforts to accommodate patients requiring emergency dental care.

  • PRIVACY AND CONFIDENTIALITY: Durban Dental Care will maintain patient confidentiality in accordance with applicable laws and regulations. Patient’s records will be kept secure and will not be disclosed with the patient’s consent, except as required by law.

  • USE OF IMAGES: Patients consent to the anonymous use of images that may be taken during treatment for marketing purposes.

  • QUALITY OF CARE: Durban Dental Care agrees to deliver dental services in a professional manner, adhering to industry standards and best practices. Patients are encouraged to provide feedback regarding their experiences to help improve the quality of care.

  • TERMINATION OF SERVICES: Either party may terminate the dentist-patient relationship at any time for any reason, with reasonable notice. In the event of termination, the practitioner will provide recommendations for alternative dental care, if necessary.

  • PATIENT SATISFACTION: If a patient is unhappy with any aspect of the treatment or service provided, they are required to notify the practice promptly. Upon receiving a complaint, Durban Dental Care will make reasonable efforts to address the issue in a timely and professional manner.

  • PROVIDER DISCRETION: Durban Dental Care reserves the right to determine the appropriateness of treatment for each individual patient, based on professional judgement, clinical assessment and ethical considerations. Practitioners may refuse to provide treatment or services that are deemed unnecessary, unethical or beyond the scope of their expertise.

  • INFORMED CONSENT: Patients have the right to receive information about proposed treatments, including risks, benefits and alternatives, to make informed decisions about their care.

  • REFUSAL OF TREATMENT: Durban Dental Care may refuse to initiate or continue treatment if: i. The patient’s medical condition poses a significant risk to their health or safety. ii. The patient fails to comply with recommended treatment plans or follow post-op care instructions. iii. The requested treatment is not within the scope of practice or expertise of the practitioner.

  • REFUNDS & COMPLETION OF TREATMENT: Once a dental treatment has been completed, no funds will be issued for services rendered. Completion of treatment is defined as the successful delivery of the agreed upon dental services, as determined by the practitioner.

  • TREATMENT OUTCOME: Patients understand that the outcome of dental treatment may vary depending on individual factors such as oral health status, compliance with post-treatment care, and other variables. Dissatisfaction with treatment outcomes does not entitle the patient to a refund for services already provided.

  • EXCEPTIONAL CIRCUMSTANCES: In rare cases where a refund may be deemed appropriate due to extenuating circumstances, such as billing errors or unforeseen complications directly attributable to the practitioner’s negligence, the decision to issue a refund will be made at the discretion of Durban Dental Care. Patients must provide documentation and evidence to support their request for a refund under exceptional circumstances.

  • PRE-TREATMENT AGREEMENT: Patients acknowledge their understanding of the no-refund policy for completed treatments before initiating any dental procedures
